Injury Waiver /
General Release

By signing your name below as a participant, and/or parent/legal guardian of participant(s) in Hyperfit Hockey 2025 Spring Tournament Teams - you acknowledge that participation in the program exposes you or your child to a possibility of serious personal injury. You, being fully aware that participation in the program exposes you to a possible risk of personal injury, hereby release Hyperfit Hockey / Hyperfit Hockey USA and its officers, directors, employees, agents, licenses, subsidiaries, consultants, independent contractors and affiliates from any and all liability from property damage, personal injuries, or other claims arising from or in connection with your participation in the program including claims that are known and unknown, foreseen and unforeseen, future or contingent. Insurance is purchased from the tournament organizer for all of these events.

Media Release

I grant permission to Hyperfit Hockey, hereinafter known as the “Media” to use my child’s image (photographs and/or video) for use in Media publications including:

  • Videos

  • Email Blasts

  • Newsletters

  • Marketing

  • Website

  • Social Media (instagram/twitter)

I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.

I am the parent or legal guardian of the below named child. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
